One Layer Up

The panel carries one reporting channel. It is a faceted glass jewel in a chrome bezel, the only element on the instrument chosen for how it looks - everything around it is plain: five binding posts, two knobs, a toggle, black paint brushed on by hand and chipped where a wrench slipped.
The lamp is wired to the output side, not to the mains. That is the correct decision, and it was not the convenient one.
There are no labels anywhere on this instrument. Whether the output is energized cannot be seen, heard, or inferred from any other element on the panel. It is the state an operator needs before connecting anything, and the builder spent his single channel reporting it. He kept the red and black posts for the same reason. He encoded what carries consequence and left everything else to be worked out.
Then the voltage knob goes to its stop, a meter across the terminals reads zero, and the lamp remains lit.
What the lamp is actually reporting
Not that the terminals are live. That the output section is energized.
Those two facts agree nearly always, which is what makes the difference invisible. The lamp is not broken and it is not lying. It reports a true condition at the layer where the wire was easy to attach - one layer above the layer the operator is standing at.
The other lamp
At Three Mile Island in 1979, a relief valve opened and failed to reseat. There was an indicator for that valve in the control room. It reported that a close command had been sent to the valve. It did not report the position of the valve, and nothing in the room did.
The command had been sent. The indicator was correct.
The operators read it as the valve is shut, because for years those two facts had agreed. Coolant left the primary loop for hours. Among the changes required afterward was an indicator reporting the valve's actual position.
The proportion
A five-dollar bench supply is not a nuclear plant. One was a student's evening project; the other was designed under federal licensing, and its failure ended commercial nuclear construction in the United States for a generation. The comparison is disproportionate on every axis that matters.
That objection is correct, and it is the finding.
The mechanism is identical at both scales, and neither instrument malfunctioned. A reporting channel was attached at the layer that was accessible rather than the layer the operator acts on. The two layers agreed during every test, every commissioning, every ordinary day of service. They disagree only in the condition the report exists to cover.
The defect cannot be caught by verifying that the indicator works. The indicator works.
Why it always drifts upward
Instrumenting the operator's layer is the expensive option, every time.
A limit switch on a valve body is harder to fit than a sense wire on a solenoid coil. A tap at the binding posts is harder to route than a tap on a rail already present in the chassis. Checking that a transaction completed is harder than checking that a service responded. Checking that records landed is harder than checking that a job started.
Cost pushes every reporting channel upward, away from the layer anyone cares about. Nothing resists it, because the readings agree - through design, through review, through years of service. The instrument is never contradicted until the day it matters, and then it is contradicted once.
There is a discipline for diagnosis that runs the other direction: look beneath the apparent break. Carrier before platform. Filesystem before framework. Network before application. The same rule applies to reporting, and almost nobody applies it there.
Instrument at the layer the operator acts on, not the layer that is convenient to sense.
The correspondence
The health check that pings the service, where the operator needs to know a transaction completed.
The sync status that reports the job started, where the operator needs to know the records landed.
The backup that reports it ran.
None of these is broken. Each returns a true value, on schedule, from one layer above the question being asked. Each will agree with the layer below it for years.
The corrected article
Move the lamp to the terminals.
That is the whole remedy. Not a label, not a second indicator, not an instruction sheet. A different attachment point for a channel that already exists, available to the builder from the same catalogue that sold him the binding posts, at no additional cost in parts.
The panel would then report the state an operator needs before connecting anything, rather than the state that was easy to sense.
Disposition
The current limit does not reach a useful upper range. Whether that is a fault or the original design is not established.
Operation was resolved in ten minutes with a multimeter, by an operator who already knew what a current limit was and that dial direction is not guaranteed. The reporting layer took three days to establish. The panel's silence cost nothing because the knowledge arrived with the operator. That is why the builder could not see the omission: he had the knowledge that concealed it.
Trust is a separate matter and remains open. It will not be settled from the panel.
